If you didn't (accidentally or on purpose) actually violate the TOS in this way, it could easily have been someone doing an adSense DOS on you. I haven't actually heard of this happening, but if I really wanted to screw with someone who used Google ads, I would set up a script to just keep generating clicks until their account gets suspended.
An adsense DOS doesn't make sense since it would show in my stats. I think they did a cross reference of IP addresses of the clicks and the ip addresses that I log in with to the account. So a DOS wouldn't make any sense unless it was a continues stream that making it obvious. And if that did happen you could just say it was some jack off.
Now if you made the script to change IPs then you would just make the person money.
